El Hogar Community Services is seeking (Mental Health Service Coordinators) to meet the needs of our community mental health programs. CORE-Marigold, CORE-Mariposa, Renew - Florin and Renew-Bercut. Each program is unique in its design and the population they serve. The CORE-Mariposa programs provide mental health and other related resources to adults 18 and older, Renew-Florin offers services to adults in the criminal justice system, and Renew-Bercut provides services to the adult population 55+ years, who require a higher level of care. Summary:  The Mental Health Service Coordinator is responsible for delivering mental health, substance abuse, and support services consistent with the principles of the Psychosocial Rehabilitation Model to an adult population with mental illness. The Mental Health Service Coordinator provides Behavioral Health recovery-based services, including, but not limited to: assessments, service plan development, case management, housing support, skills training, group facilitation, crisis intervention, and individual and group therapy. The Mental Health Service Coordinator may be asked to carry a caseload of 75+ clients in the field and maintain contact as defined by the Program Contract(s).

Essential Duties and Responsibilities:

• Support and carry a caseload.  Caseload size varies based on program contract and programmatic needs.
• Assessments : Assess and gather client information for domain-based assessments to evaluate the mental health needs of clients, considering factors such as medical history, psychosocial background, and presenting symptoms. Screen for housing, benefits, and community needs. Other assessments may include using the Strengths Based Model Assessment, ANSA, or other screening tools based on program requirements
• Treatment Planning : Develop and implement individualized treatment and Personal Empowerment plans in collaboration with clients that incorporate evidence-based therapeutic interventions to address clients’ specific needs. As needed, support and develop a housing plan with the client to address housing challenges, goals, and supportive interventions.
• Case Management : Assist clients in navigating community resources, social services, housing, and financial assistance programs. Advocate for clients to ensure they receive appropriate and timely services.
• Crisis Intervention : Manage and respond to crises effectively, employing crisis intervention techniques to ensure the safety and well-being of clients experiencing acute emotional distress. Actively participate in debriefing sessions with the team to share experiences and enhance crisis response strategies.
• Progress Monitoring : Meet with clients regularly per client needs and contract obligations. Regularly review and assess clients’ progress toward treatment goals, adjusting social rehabilitation and case management services to achieve positive outcomes.
• Documentation : Per El Hogar and Sacramento County regulatory documentation policies & procedures, maintain accurate and timely documentation of client assessments, progress notes, treatment plans, and other relevant records in compliance with regulatory and organizational requirements (e.g., progress notes, adverse incident reports, discharge summaries, Medi-Cal billing, EHR forms, HMIS, etc.).
• • Responsible for producing the required number of service units per month (at least 50% of staff’s time must be spent providing face-to-face or other eligible community-based support with/for clients) .
• Collaboration : Able to work independently and with a multi-disciplinary team of professionals, including med providers, mental health clinicians, peers, care coordinators, and other members, to discuss/develop/facilitate integrated care plans for clients. Collaborate with external service providers to ensure holistic support for clients.
• Education and Support : Educate clients and their support persons about mental health conditions, coping strategies, and community resources to enhance their overall well-being and empower them to participate in their treatment process actively.
• Professional Development : Pursue ongoing professional development opportunities to stay informed about the latest developments in mental health care. Actively engage in training sessions, workshops, and conferences to enhance skills and knowledge. Share acquired knowledge with the team, contributing to a culture of continuous learning. Participates in the internal and external county utilization review of client charts.

• Ethics and Confidentiality : Adhere to strict ethical standards and maintain client confidentiality per legal and professional guidelines. Ensure the protection and privacy of all Protected Health Information (PHI) by following all Agency policies and procedures.

Supervisory Responsibilities:

This position has no supervisory responsibilities.

Education and Experience:

Education and/or Experience: Minimum qualifications can be met in the following ways:

High School Diploma (Required);

Associate’s, Bachelor’s, or Master’s Degree from an accredited or regionally approved college

or university in the area of social work, psychology, vocational rehabilitation, criminal justice, or a related field (Preferred);

Experience in mental health social services (Required); Work experience with adults who have mental health/substance abuse/homeless/ forensic behavioral population (Preferred).

Certificates, Licenses, Registrations:

· Current, Valid California Driver's License and driving record acceptable to El Hogar and El Hogar's auto insurance carrier. Current proof of personal auto insurance (Required).

·        Current valid CPR training.

·        El Hogar provides the following employee benefits: (The benefits listed below are based on an employee who works Full-time 40 hours/week. An employee who works less than 40 hours but more than 20 hours is eligible for holiday, vacation, and sick days on a pro-rate rate) :
