Job Summary Under the general supervision of the medical director of Florida PACE Centers, provides direct primary medical care treatment, and counsel to all participants enrolled into the program. Essential Job Functions Perform comprehensive history and physical on new referrals; perform interval history and physical on established participants quarterly or as indicated. Manage patients with acute, episodic, and chronic illnesses. Assume ongoing responsibility for each patient under care. Refer participants to medical director or medical specialist as indicated. Admit participants to the hospital, provide primary care in conjunction with Medical Director, provide updates to multidisciplinary team on a regular basis, judiciously utilize special consultants and coordinate discharge planning with team. Manage care of participants in the nursing home through regular visits, telephone contact with nursing home staff and admit nursing home participants to hospital when necessary. Actively participate in family conferences, attend interdisciplinary team meetings and contribute to the team. Participate in committees and work groups as assigned. Document treatment in patient charts according to established department guidelines. Job Requirements Education Graduate from an accredited school of nursing. Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA). Experience 2-3 years’ experience preferred; minimum of 1 year specifically working with frail elderly population. Licenses/Certifications Current ARNP/PA license by the Florida State Board of Nursing. Abilities Knowledge of nursing care practices for rehabilitation, geriatric and dementia patient populations preferred. Demonstrates a caring attitude toward residents, patients, family members, employees, and other facility guests. Environment and Work Conditions Normal working condition with adequate lighting and ventilation. Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas. Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds. Routine exposure to blood/body fluids/tissues.
