Job Summary:

Provides comprehensive diabetes self-management education and medical nutrition therapy to individuals and groups. Collaborates with healthcare teams to develop personalized care plans aimed at optimizing diabetes control and enhancing the patient's quality of life.

Responsibilities:

• 
  Deliver diabetes education and nutritional counseling tailored to individual needs.
  
  
• 
  Assess patients educational requirements, lifestyle, and health goals to develop effective intervention strategies.
  
  
• 
  Coordinate with healthcare providers to create and implement personalized treatment protocols.
  
  
• 
  Manage and utilize diabetes monitoring technology, including glucose meters and insulin devices.
  
  
• 
  Educate patients on medication use, device management, and lifestyle modifications for diabetes control.
  
  
• 
  Document patient progress and update electronic medical records accurately using EPIC and other software.
  
  

Qualifications:

• 
  Hold a Bachelor's degree in Dietetics or related field.
  
  
• 
  Obtain CDCES or CDE certification; BLS certification preferred.
  
  
• 
  Demonstrate experience in diabetes and gestational diabetes management.
  
  
• 
  Possess proficiency in using diabetes monitoring devices and computer applications like Microsoft Office and EPIC.
  
  
• 
  Exhibit advanced knowledge of adult learning principles and effective teaching techniques.
  
  
• 
  Show excellent organizational skills and attention to detail in a clinical setting.
