Position: Registered Dietitian Diabetes Educator

Location: TBD, City, State

Shift:

• Day 5x8-Hour (07:00 - 15:30)
• Day 5x8-Hour (10:00 - 18:30)

Job Description:

The Registered Dietitian Diabetes Educator is responsible for providing comprehensive diabetes self-management education and training (DSMT) as well as medical nutrition therapy (MNT) to individuals, families, and groups. This role involves managing care for a distinct patient population through assessment of educational needs, nutritional guidelines, and lifestyle interventions. The educator will work collaboratively with other healthcare professionals, including physicians and social workers, to develop effective individualized care plans that empower patients to control their condition, avoid complications, and improve their quality of life.

Key Responsibilities:

• Provide diabetes education and training to patients and their families.
• Assess educational and nutritional needs of patients.
• Develop and implement individualized care plans in collaboration with healthcare providers.
• Guide patients in techniques and behavioral modifications to manage their health.
• Utilize diabetes monitoring equipment and related software effectively.
• Contribute to interdisciplinary team efforts to enhance patient care.

Education and Qualifications:

• Bachelor's degree in Dietetics or a related field.
• Certification as a Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) preferred; candidates in the process of obtaining certification may be considered.
• Basic Life Support (BLS) certification required.

Skills Required:

• Background in diabetes management and gestational diabetes.
• Proven clinical judgment and technical skills within diabetes education.
• Expertise in diabetes monitoring and managing equipment, such as blood glucose monitors and insulin delivery devices.
• Strong knowledge of adult learning theory and group education methodologies.
• Exceptional attention to detail, time management, and organizational skills.
• Proficiency in Microsoft Office Suite and electronic medical record (EMR) systems.

Patient Care Focus:

The role requires a commitment to providing exceptional patient care, with an understanding of various diabetes-related diagnoses, including but not limited to:

• Gestational diabetes
• Type 1 and Type 2 diabetes
• Prediabetes
• Hyperglycemia and insulin resistance

Additional Responsibilities:

• Assist with technology training for patients.
• Support unit education initiatives for staff onboarding and annual competencies.
• Participate in quality improvement efforts for diabetes management.
