Responsibilities
- Conduct comprehensive physical therapy evaluations for residents and develop individualized treatment plans aligned with short- and long-term goals.
- Deliver evidence-based interventions to improve mobility, strength, balance, gait, and functional independence; tailor therapies to the LTC population.
- Reassess progress regularly, adjust plans, and document patient care in the medical record per facility standards.
- Collaborate with an interdisciplinary care team (nursing, therapy colleagues, social work, and other staff) to optimize resident outcomes and discharge planning.
- Educate residents and family members on therapeutic exercises, safety strategies, fall prevention, and home exercise programs.
- Maintain safety, infection control, and compliance with LTC policies; prioritize patient comfort and dignity while promoting functional gains.

Requirements
- Active physical therapy license in Montana or eligible for licensure
- Graduation from an accredited physical therapy program (DPT preferred)
- Ability to work in a long-term care setting and meet travel assignment requirements
- Strong communication and teamwork skills; ability to document and collaborate within an interdisciplinary team
- Commitment to resident-centered care and adherence to professional standards and facility policies
