Our mission is to improve patient outcomes by delivering proactive, relationship-based healthcare that keeps patients healthier at home, reduces avoidable hospitalizations, and enhances quality of life.As we continue expanding our Population Health programs, we are seeking an experienced and compassionate Nurse Practitioner to join our growing interdisciplinary clinical team.Position SummaryThis is a unique opportunity for a Nurse Practitioner who enjoys building long-term patient relationships through proactive, technology-enabled care.This position is centered on providing advanced clinical oversight for patients enrolled in Vanguard Mobile Physicians' Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Population Health programs. Working alongside an experienced team of care coordinators, physicians, home health clinicians, hospice, and palliative care providers, you'll help identify changes in patient condition early, optimize treatment plans, and improve outcomes before hospitalization becomes necessary. This is not a high-volume urgent care telemedicine position.
