In this role, you will be responsible for the oversight and supervision of the interdisciplinary team of professionals, including nurses, social workers, therapists, and home health aides. The Clinical Team Manager oversees and supervises the day‑to‑day activities, ensuring quality client care is provided and monitoring compliance with organizational standards and federal/state regulations related to health care delivery. Responsibilities Assign new client admissions to RN Case Managers and oversee clinical self‑scheduling processes ensuring equal distribution of client assignments and scheduling. Review staff documentation, including OASIS, to ensure compliance. Perform one‑on‑one case conference reviews and mentor staff appropriately. Assist field clinicians as necessary. Assist with quality review as needed. Supervise, train, assure adequate staffing levels and evaluate performance. Monitor quality of care provided and completion of charting. Team player mentality. Education And Experience Licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) in Michigan. At least 2 years of home health experience.
