Looking for a change of scenery and a chance to help others. Join a skilled nursing facility in Chandler, AZ as a Physical Therapist where you will help patients recover from injuries and manage chronic conditions while working in a senior living community setting. The role involves treating residents in outpatient clinics located within assisted living and memory care neighborhoods.

Minimum Qualifications

• Physical Therapist (PT) degree from an accredited program (Bachelor's, Master's, or Doctoral degree as applicable).
• Active Arizona PT license required prior to the start of assignment.
• BLS certification from AHA or ARC may be required.
• Experience or comfort working with late-stage dementia or locked memory care populations preferred.
• Ability to work independently and flexibly across two clinic locations.
• Experience with EMR systems; familiarity with Optima or similar systems (Net Health-like) preferred.
• Productivity expectations: PT target approximately 75%.
• New graduate PTs will be considered.

Preferred Qualifications

• Prior experience in skilled nursing, outpatient geriatric settings, or senior living communities.
• Experience managing challenging behaviors associated with dementia.
• Experience floating between multiple sites or covering split schedules.

Assignment Details

• Shift schedule: Monday through Friday (specific days: M-F).
• Clinic split between two locations approximately 15 minutes apart; candidates should be willing to float between sites on rare occasions.
• Orientation: Clinic Director onsite first week and available several days per week thereafter.
• Setting: Outpatient clinics embedded within an assisted living and a locked memory care unit.

The position supports mobility, fall prevention, and functional independence for older adults living in assisted living and memory care neighborhoods. Clinicians will deliver individualized treatment plans, collaborate with interdisciplinary teams, and document progress in the clinic EMR to ensure continuity of care for residents.
