Registered Nurse (RN) - Case Management

Registered Nurse Case Manager
Monday - Friday, Day 5x8-Hour (08:00 - 16:30)
Assignment Details
Maximum of 7 days RTO. If one major holiday falls within the assigned period, the traveler is expected to work it. If two major holidays fall during the assignment, both must be worked. A two-week pushback for orientation is required if compliance is not met.
Requirements
- Minimum of 3 years of experience required.
- Unit does not accept first-time travelers.
- Clinical screening required to determine fit.
- Full compliance required to start on time.
Responsibilities
- Assess, evaluate, and drive discharge plans effectively.
- Coordinate services to ensure appropriate level of care.
- Initiate, evaluate, and re-evaluate treatment plans daily.
- Monitor patient progress daily and identify barriers to discharge.
- Adhere to regulatory guidelines regarding patient choice and notices.
