Registered Nurse

The RN implements professional nursing practice centered on the patient as a whole person. This role focuses on the creation of a caring environment aimed at healing and wellness. Nursing practices are based on evidence, current research, hospital policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit, including provision of nursing rounds, assessments, and treatments. Facilitates admission, transfer, and discharge activities. Provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. Performs and documents a comprehensive assessment according to RN scope of practice and hospital policies. Considers the patient's culture, age, and developmental level. Identifies patient risk factors and presenting complaints. Obtains patient health information from family or others as necessary. Collects objective and subjective data and verifies assessments obtained by other healthcare team members. Develops a problem list and prioritizes patient needs based on input from the inter-professional team. Designs actions and interventions relevant to the patient's culture and age. Determines desired outcomes in coordination with the patient and family according to the medical plan of care. Implements patient care therapies and procedures with empowerment and clinical autonomy within scope of practice. Explains procedures, treatments, and processes to patients and families. Initiates actions to alleviate the patient's stress and discomfort. Recognizes, responds to, and communicates changes in patient condition appropriately. Conducts and documents ongoing assessments in accordance with hospital policy and established standards. Delegates tasks appropriately and oversees team members in patient care activities. Monitors accuracy and completeness of delegated patient care provided by team members. Identifies and addresses educational needs of patients according to their comprehension level. Facilitates the patient discharge process by executing discharge orders timely and coordinating with Case Management. Provides input into patient/nurse assignments considering staff availability, unit activity, and patient acuity. Actively participates in unit-based activities supporting organizational values and priorities. Promotes a positive group dynamic and healing environment through words and actions. Assures nursing excellence in accordance with accepted standards, evidence-based practice, and research. Participates in department and unit quality improvement activities including patient satisfaction, patient falls, and pressure ulcers. Uses a holistic perspective regarding patient care.
