Family Practice/Primary Care Nurse Practitioner

Position Overview

We are seeking a Board Certified Nurse Practitioner for an Institutional Special Needs Plan (ISNP) mobile position providing geriatric primary care to patients in their homes and long term care facilities within the San Fernando Valley. This role is ideal for mission driven NPs who value autonomy and relationship focused care.

Location: San Fernando Valley (Burbank Santa Clarita West Hills)
Position Type: Locums (6 months, extension possible)
Start Date: ASAP
Schedule: Monday Friday, 8 AM 5 PM
Setting: Mobile/Travel NP patient homes & long term care facilities
Patient Volume: 5 7 patients/day
EMR: Athena
Support: Virtual physician oversight

Key Responsibilities

Deliver comprehensive geriatric primary care in home & facility settings
Manage a patient panel of across 20 facilities
Perform assessments, diagnosis, treatment planning, and follow up
Coordinate patient care with virtual physician support
Ensure compliance with HEDIS quality measures & Value Based Care models
Document using Athena EMR

Required Qualifications

Board Certified NP
Active CA license (or willingness to obtain)
BLS & DEA
1+ year experience in geriatric primary care
Experience in travel/mobile NP settings preferred
Must have working knowledge of HEDIS and VBC

Position Details

Contract length: 6 months + possible extension
Travel/Lodging: All inclusive
Compensation: Open
On Call: None
Orientation: 1 week
Onboarding: 2 3 weeks
COVID Vaccine: Not required
Interview: Virtual with Regional Medical Director
