Collaborate with an interdisciplinary team to optimize independence in activities of daily living and ensure safe transitions to next levels of care. Responsibilities Screen and evaluate patients to identify functional impairments and goals. Develop and implement OT treatment plans focused on ADLs, mobility, and arm/hand function as appropriate. Provide therapeutic activities and modalities to improve function and prevent deconditioning. Educate patients and families on home exercise programs and safety strategies. Document progress, goals, and outcomes in the EHR; communicate with the care team. Plan for discharge and coordinate with case management, nursing, and therapy teams. Qualifications Education: Graduate of an accredited Occupational Therapy Assistant program. Licensure: Currently certified as a COTA in the United States; licensure requirements vary by state. Certifications: Basic Life Support (BLS) certification Experience or Skills: Experience in acute care rehabilitation or hospital-based therapy with strong documentation and teamwork abilities. Experience 1+ year of OT practice in acute care or hospital settings Experience evaluating and treating adults with medical conditions requiring inpatient rehab Preferred Experience with hand therapy or wheelchair mobility assessments Knowledge of inpatient discharge planning processes Work Environment Acute care hospital environment with collaboration among physicians, nurses, and therapists. Daily therapy participation with emphasis on early mobilization and safe discharge planning; documentation in the EHR and care coordination with case management.
