Are you an experienced Social Services professional who wants a leadership role where your work truly matters? We are looking for a Director of Social Services to join our skilled nursing team and become an important advocate for our residents and their families.

What You'll Do

As our Director of Social Services, you will:

• Lead and oversee the Social Services Department.

• Advocate for residents and ensure their emotional and psychosocial needs are being met.

• Build strong relationships with residents and their families.

• Help residents and families navigate the transition into long-term care.

• Participate in care planning and discharge planning.

• Coordinate transitions of care and help connect residents with appropriate resources.

• Partner with nursing, therapy, activities, dietary, and other departments as part of the interdisciplinary team.

• Maintain accurate assessments, documentation, progress notes, and care plans.

• Ensure compliance with state, federal, and facility requirements.

• Provide leadership, support, and guidance to social services staff.

What We're Looking For

• BSW required; Master's degree preferred

• Current state license or certification (LSW or equivalent) in good standing

• Skilled nursing facility experience required

• Ability to work approximately 35–38 hours per week

• Strong communication, organization, and leadership skills

• Compassionate and resident-focused approach

• Ability to work effectively with residents, families, and an interdisciplinary team
