• Locations include: Newport News, Hampton, Kilmarnock, Gloucester, Yorktown and Southside, VA

$50-$60/hour  (all inclusive)

Guided by our core values of Passion, Respect, and Excellence , the Nurse Practitioner provides advanced clinical expertise in hospice and palliative care, ensuring comfort, dignity, and individualized support for patients and their loved ones.

Responsibilities for Nurse Practitioner:

• Provide medical support and coordination to hospice patients and family members.

• Provide medical expertise in pain and symptom management and evaluate terminal prognosis for patients, families, and staff.

• Ensure care delivery aligns with established hospice and palliative care guidelines, standards, and best practices.

• Regularly communicate and collaborate with VHS providers regarding new patient referrals.

• Perform patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, and other settings, including telehealth.

• Collaborate with attending physicians regarding patient care and treatment, and support the coordination of hospice referrals.

• Provide the Hospice Medical Director with the required face-to-face encounter documentation.

• Perform other duties as assigned by the Medical Director.

Qualifications for Nurse Practitioner:

• MSN from an accredited nursing school required; current state licensure as a Registered Nurse and Nurse Practitioner in good standing required in the state of Virginia.

• 2–4 years of experience in a medical specialty involving care of patients diagnosed with serious, advanced, or terminal illnesses is required.

• Minimum of 2 years of clinical experience in Oncology, Pain & Symptom Management, Palliative Care, Hospice, hospital, or physician practice setting required.

• Current CPR Certification required.

• Federal Controlled Substance Registration (DEA) and Malpractice Insurance Certificate

• Ability to obtain hospital privileges as required.

• Certification by the American Academy of Nurse Practitioners and/or the National Board for Certification of Hospice and Palliative Nurses (NBCHPN) preferred; certification required within an agreed-upon timeframe if not currently obtained.

• Valid driver’s license, personal vehicle for on-the-job transportation, and proof of insurance as required per policy.
