PT - Physical Therapist

Home Health Registered Nurse (RN)

This is a 13-week contract position.

Day shift; 5x8 hours (08:00 - 17:00).

We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide exceptional care to our patients in a home setting. The ideal candidate will be responsible for executing personalized patient care plans, supporting patients' health needs, and ensuring the highest quality of care.

Responsibilities:

Conduct thorough assessments of patients' physical, emotional, and psychological needs.
Develop and implement comprehensive care plans in collaboration with the healthcare team.
Administer medications and monitor patient progress.
Educate patients and families about health management and safety precautions.
Utilize various medical equipment and devices necessary for patient care.
Document patient interactions and care provided accurately.
Perform skilled home care visits, ensuring necessary documentation and billing accuracy.

Required Experience:

Must have at least 1 year of home health experience.
OASIS training preferred.
Experience with various patient conditions including med/surg, cardiac, orthopedic, and respiratory.

Certifications & Licensure:

Active RN license in the state or a compact license.
Current Basic Life Support (BLS) certification.
