About the Role:

Village Choice Healthcare Inc is looking for a compassionate and dedicated Home Health Nurse to join our growing team in Beaumont, TX. Responsibilities:

Conduct in-home patient assessments, evaluations, and develop individualized care plans

Administer medications, wound care, IV therapy, and other skilled nursing treatments

Monitor and document patient progress, vital signs, and changes in condition

Coordinate care with physicians, therapists, and other healthcare team members

Educate patients and family members on disease management, medications, and home safety

Maintain accurate and timely clinical documentation in compliance with regulatory standards

Provide compassionate end-of-life and chronic disease management support as needed

Requirements:

Active RN or LVN license in the state of Texas

Previous home health, hospice, or community nursing experience preferred

Strong clinical assessment and critical thinking skills

Excellent communication and patient education abilities

Valid driver's license and reliable transportation for home visits

Proficiency with electronic health records (EHR) and clinical documentation

Ability to work independently and manage a patient caseload efficiently
