Key Responsibilities

• Provide skilled physical therapy assistant services to patients in a home health setting
• Manage a geriatric-focused caseload, visiting up to 6 patients per day
• Maintain productivity standards of 30 productivity points per standard workday
• Document patient care accurately and in a timely manner using HCHB electronic medical records
• Travel independently to patient homes within the service area, with occasional visits extending beyond a 50-mile radius

Required Qualifications

• Active California Physical Therapy Assistant (PTA) license
• Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
• Proficiency with HCHB electronic medical records system
• Current Basic Cardiac Life Support (BCLS) certification
• Reliable personal transportation for patient home visits

Preferred Experience / Skills

• Experience working with geriatric patient populations in a home health environment
• Familiarity with efficient caseload and route management in a field-based setting
