How You'll Make a Difference As a Registered Nurse (RN) the work you do every day makes a difference in the lives of our patients. You will provide one on one patient care, work at the top of your license, and use leadership skills to coordinate care and deliver home-based nursing care as directed by an attending physician. How You'll Work You’ll provide skilled nursing care in a patient’s home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You’ll be responsible for all practices and duties within the scope of practice outlined by the state. Major Areas of Responsibility Plan of Care: Observe and monitor patient conditions; perform OASIS assessments to develop and adjust individualized care plans. Minimize Patient Risk: Administer medication as prescribed and prioritize visits for high‑risk patients to reduce rehospitalizations. Continuity of Care: Manage multidisciplinary care and promote continuity with admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families using teach‑back methods to ensure understanding. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaboration: Work with the interdisciplinary team and physicians to provide optimal care. Compliance: Understand and follow agency policies, procedures, rules, and regulations; communicate schedule changes. Operations: Attend in‑service training and mandatory agency meetings. Requirements Registered Nurse with current license in the state of employment. Minimum one‑year experience as an RN in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in your name as a driver. Capable of meeting all physical demands of the role.
