RN Patient Care Manager - Norwood

Schedule: Full-time | Monday - Friday | 8 a.m. to 5 p.m. Key Responsibilities This PCM position is leading the in field clinical team Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements Coordinate and monitor patient care delivery Supervise clinical staff to ensure consistent, appropriate service Maintain clear communication with patients, caregivers, referral sources, and field and office staff Drive quality outcomes by meeting department goals and supporting improvement initiatives Model professionalism and organizational values in daily work Uphold compliance through required training, accurate reporting, and cooperation with audits Promote a safe work environment by following established safety protocols Support staff growth through coaching and continuing education Patient Care Manager Qualifications Must have a License in the state or an expired licence from the state that can be reinstated would work. Previous experience in home care setting with two years management or supervisory experience, preferred.
