Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care (LTC)

Job Description

This role involves providing essential nursing care to residents in long-term care settings. Responsibilities include monitoring residents' mental and physical well-being, administering medications, and collaborating with a multidisciplinary team to ensure optimal patient care.

Key Responsibilities

• Continuously observe residents for changes in mental and physical health and report findings to the Supervisor.
• Accurately receive, transcribe, and implement physician's orders while documenting through an electronic system of MARs/TARs.
• Administer medications and treatments as prescribed, ensuring proper documentation of all activities.
• Supervise nursing assistants as per licensing requirements and assist with resident care when time permits.
• Accompany physicians during resident rounds to facilitate effective communication regarding care needs.
• Administer oxygen and provide enteral feedings as per protocols; perform wound care and manage wound vacs.
• Manage administrative duties, including checking appointments and ensuring compliance with nutritional and therapeutic documentation.
• Assist in the admission, discharge, and transfer of residents while maintaining accurate records.
• Communicate effectively with residents' families regarding care updates and obtain necessary consents.
• Oversee nursing staff during shifts, ensuring compliance with state and federal regulations for medication management.
• Support the orientation and ongoing education of new nursing employees, promoting a culture of continuous improvement.

Qualifications

• Minimum of 3 years of current experience in a similar role within the last 6 months.
• Willingness to float to various units as needed.
• IV certification is required; candidates must be willing to pursue certification during onboarding if not already certified.
• Active New Hampshire nursing license at the time of submission.
• Current BLS certification (American Heart Association).
• COVID vaccination is mandatory.

Additional Information

Candidates should be prepared for rural living conditions with limited housing options and must possess their own transportation. Weekend work is required every other weekend. No on-call requirements are included in this position.
