Join a physician-led primary care organization focused on value-based Medicare Advantage, where outcomes are prioritized over patient volume. This role offers a manageable patient panel and robust care team support, enabling you to practice medicine with ample time and purpose. The organization emphasizes quality, patient experience, and physician wellness, fostering a relationship-driven care model. Practice Info Value-based, full-risk Medicare Advantage model Patient panel capped at ~450 patients Focus on prevention, care coordination, and behavior change Dedicated care teams (MA, Care Coordinator, Care Facilitator) Emphasis on quality, patient experience, and physician wellness Appointment utilization (18–21 slots per day) Responsibilities Provide comprehensive primary care to older adult patients Maintain full accountability for clinical outcomes and patient experience Serve as a health coach, guiding lifestyle and long-term health improvement Coordinate specialty and hospital care while staying actively involved Lead and collaborate with an interdisciplinary care team Supervise and remain accountable for Advanced Practice Providers (APPs) Partner with medical and operational leadership to meet quality and cost goals Compensation Salary: $250,000 – $306,714 (based on experience) Benefits Comprehensive benefits package Paid time off and CME Long-term career growth and leadership opportunities Strong work-life balance Shift & Schedule Predictable schedule Requirements MD or DO required Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics Active or eligible state medical license Active DEA Commitment to value-based, team-oriented care
