Position Overview

This role coordinates all aspects of patient care, both inpatient and outpatient, in alignment with program assignments. Key responsibilities include record review, medication refills, and prior authorizations. You will also provide patient education and assist with the review of test, procedure, and lab results.

Responsibilities

• Coordinate medically necessary referrals for services, consultations, equipment, and supplies.
• Act as a liaison between administrative and clinical staff and physicians within the transplant department.
• Support patient discharge planning as needed.
• Provide education to patients regarding their care and medications.
• Assist patients and/or physicians in clinical settings and on hospital floors as required.
• Carry out patient care orders as directed by physicians, adhering to hospital policies.
• Assist with patient admissions and discharges as required.

Requirements

• Basic Life Support (BLS) certification.
• California RN Licensure.
• Bachelor's degree in a relevant field.
• Experience with EPIC Phoenix charting system is mandatory.
• Outpatient experience in EPIC is essential.

Experience and Skills

• At least 2 years of relevant experience.
• Proficiency in medication reconciliation, interpretation of laboratory results, and vital sign assessment.
• Understanding of patient health questionnaires and histories.
• Experience with transplant populations is required.

Working Conditions

• Schedule: Monday to Friday, 5x8 hour shifts (8:00 AM - 3:30 PM).
• No weekend or holiday shifts required.

Additional Information

This position is responsible for coordinating care for pre and post-transplant and disease management patients, including medical management and discharge planning. Participation in educational programs about transplantation for various stakeholders is expected.

Compliance and Onboarding

• Completion of required labs within 3-5 days of contract signing.
• Provide a 7-year work history verification.
• Mandatory completion of onboarding modules.
