**Home Health Caregiver**

**Job Summary:**

As a Home Health Caregiver with BrightCare HomeCare, you will play a vital role in providing compassionate and personalized care to individuals in their homes. Our team is dedicated to delivering high-quality services that cater to the unique needs and goals of each client. As a Home Health Caregiver, you will be responsible for providing direct patient care, maintaining a clean and safe environment, and collaborating with our multidisciplinary team to ensure the best possible outcomes.

**Responsibilities:**

* Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting
* Maintain accurate and detailed records of patient care and progress
* Implement and update patient care plans in collaboration with the healthcare team
* Assist with medication administration, wound care, and other medical treatments as needed
* Maintain a clean and safe environment, including laundry, housekeeping, and meal preparation
* Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
* Collaborate with our multidisciplinary team to develop and implement individualized care plans
* Participate in continuous quality improvement initiatives to enhance patient care and outcomes

**Requirements:**

* **High school diploma or equivalent required**
* **Certification in home health care or equivalent experience**
* **Ability to lift, transfer, and reposition patients, as needed**
* **Effective communication and interpersonal skills**
* **Ability to work independently and as part of a team**

**Work Environment:**

* Work in clients' homes in the Gretna, Louisiana area
* Must be able to travel to and from client homes, as needed
* Ability to work varied shifts, including days, evenings, and weekends
