Job Title: Home Health RN Location: Worth, Illinois (serving Justice, Bellwood, and surrounding areas) Duration: Ongoing Hours: 40 hours per week (8‑hour shifts) Employment Type: Staff Responsibilities Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager. Deliver quality patient care in accordance with care plans. Comply with charting and documentation deadlines. Self‑manage time and schedule; travel approximately 20% of the day to patient locations. Utilize tablets, mobile phones, and EMR software for patient care. Qualifications RN license in the state of employment. Graduate of an approved school of practical nursing. Minimum one year of RN experience with demonstrated clinical competence. Current driver’s license. Demonstrated experience with tablets, mobile phones, and EMR software.
