The RN provides care to these patients via the utilization of the nursing process, nursing trends, technology, financial and human resources within the philosophy of the institution. Essential functions include performing all responsibilities and duties required by the unit specific to the scope of service. The RN implements procedures and theories related to the specific area of practice. Maintaining up-to-date and accurate documentation of nursing care is essential to assure the integration of information for use by the healthcare team. The RN completes an initial assessment upon patient admission and develops an appropriate plan of care in accordance with unit standards that is based on patient and family needs. The RN implements the plan of care, nursing interventions, and patient care procedures. The RN modifies the plan of care based upon continuous evaluation. Taking responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents is crucial. The RN communicates with others directly and in private when necessary to resolve issues. Offering constructive feedback to assist in the professional development of peers is encouraged, as is conferring with colleagues to expand knowledge base. Identifying personal learning needs and taking initiative to begin appropriate solutions is highly valued. Fulfilling all requirements of departmental competencies is mandatory. Department-specific job functions include receiving patient bedside shift report from another RN at the beginning of each shift. Ensuring the inclusion of patient input, visual validation of a safe patient room environment at the time of shift change, and updating of patient communication boards in each patient room is required. The RN delegates patient care tasks/responsibilities to assigned healthcare team members and validates completion of delegated tasks throughout the shift. Completing and documenting basic RN patient care activities such as assessments, interpretation of vital signs, ensuring physician order completion, admission/discharge of patients, hourly rounding on each assigned patient, care plan documentation, and patient education is expected. Meeting documentation standards for each assigned patient is essential. The RN provides patient medications utilizing the patient five rights: right patient, right medication, right route, right dose, and right time. Maintaining patient safety as the highest priority level, including safety measure compliance such as fall precautions, pressure ulcer prevention, isolation precautions, and patient ambulation safety is critical. The RN is expected to maintain a positive attitude on the unit to promote a healthy work environment and a healing patient environment. Keeping a diligent focus on promptly answering patient call lights to ensure effective triage and completion of patient requests and medical needs is necessary. Using critical thinking to evaluate the overall patient condition and using nursing judgment to identify and act on changes in patient status is important. Effectively communicating within the healthcare team promotes interdisciplinary collaboration, effective patient care coordination, and patient safety. The RN effectively manages time to enable a quick admission/discharge process for observation status patients and facilitates completion of patient testing to move patients swiftly towards discharge disposition.
