Position Summary The Licensed Practical Nurse (LPN) plays a key role in delivering high‑quality, in‑scope nursing care as part of an interdisciplinary medical team. This position supports patient assessments, clinical procedures, education, and care coordination to ensure a positive and safe patient experience. Key Responsibilities Assist physicians and RNs with patient examinations and treatments. Administer medications and injections as prescribed. Obtain and monitor vital signs, complete medication reconciliation, and document clinical updates. Perform routine procedures such as lab specimen collection and EKGs. Conduct nurse visits, including wound care, IV therapy, vaccinations, BP checks, patient education, oxygen walk‑tests, EKGs, and catheter care. Provide patient and family education on conditions, treatments, and preventive care. Conduct health screenings, immunizations, and focused assessments. Support scheduling of appointments, procedures, and follow‑up care. Coordinate referrals to outside providers and facilities. Maintain accurate and timely medical documentation. Collaborate with the interdisciplinary care team to support comprehensive patient care. Participate in training, quality improvement, and professional development activities. Foster a safe, welcoming, and patient‑centered clinic environment. Uphold confidentiality, professionalism, and ethical standards. Required Qualifications Completion of an accredited LPN program. High School Diploma or equivalent. Eligible to provide services under federal healthcare programs (not excluded by OIG/GSA). Proficiency with electronic health records and basic computer applications. Strong organizational, multitasking, and communication skills. Ability to work independently and collaboratively. Proficiency in written and spoken English. Ability to meet KHP immunization requirements. Ability to pass a background check and drug screen. BLS certification (or ability to obtain within 180 days of hire). Preferred Qualifications Bilingual in English and Spanish.
