Remote Care Navigator: Grow into a Care Advocate

Wider Circle is seeking a Care Navigator to join our Care Delivery Operations. In this entry-level role, you'll talk with patients by phone to complete health assessments and intake appointments, building trust and guiding them through initial steps. You will document notes while learning the process, coordinate with Healthie, and learn to connect patients to community resources.
