MDS Coordinator (RN) - Santa Maria, CA

At Santa Maria Post Acute, we strive to provide the best quality clinical care to our residents. We are seeking a dedicated and compassionate MDS Coordinator (RN) to join our team.

Job Summary

As a MDS Coordinator (RN), you will conduct, collect data, and coordinate the development and completion of the resident assessment in accordance with federal, state, and local laws, guidelines, and regulations governing the facility. You will collaborate with all levels of nursing personnel, facility consultants, physicians, and department managers to ensure accurate and complete resident assessments.

Responsibilities

• Conduct and collect information for the development and completion of the minimum data set (MDS) in accordance with current rules, regulations, and guidelines

• Assist in the implementation of care area triggers and resident care plans

• Participate in facility surveys made by authorized government agencies

• Work with the Interdisciplinary Team (IDT) developing a comprehensive resident assessment and care plan for each resident

• Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or to amend the admission assessment

• Contact and schedule participation by outside members of the care plan team, including the resident's representative and/or other interested family members

• Participate in planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility

• Ensure accuracy and completeness of assessment functions and awareness of penalties for false certification

• Assist in coordinating the development of a written plan of care for each resident, including measurable objectives and timetables

• Ensure timely completion of initial resident assessments and comprehensive care plans

• Assist the nursing staff in encouraging resident and family participation in the development and review of the resident's plan of care

• Ensure timely transmission of MDS assessments to appropriate agencies

Requirements

• Current state licensure and registration as a registered nurse

• Completed nursing program and degree from an accredited college or university

• Preferably a minimum of one year in a resident assessment position

• Progressive nursing experience with demonstrated leadership and teaching abilities

• Strong communication skills, both oral and written

• Knowledge and awareness of the care needs of the elderly and/or chronically ill

• Knowledge and ability to teach adult learners

• Basic computer skills

• Ability to organize, analyze, investigate, and implement principles of infection control committee
