Key Responsibilities\n

• Collaborate with a multidisciplinary treatment team to deliver coordinated care services tailored to the unique needs of clients and their families.

• Document client progress accurately and maintain reliable attendance to fulfill essential duties effectively.

• Provide individualized case management, which includes assessing client needs, developing treatment plans, and connecting families to appropriate resources.

• Deliver services within community settings or in the office, ensuring adherence to all contractual obligations set forth by funding sources.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Self Restoration Counseling Services, you will work with a diverse clientele ranging from infants to older adults. Qualifications\n
