House Calls Provider – Nurse Practitioner / Physician Assistant

Location: Virginia | Field-Based
Employment Type: Full-Time
Reports To: Medical Director (Clinical) | Practice Administrator (Operations)

What You’ll Do

• Provide in-home primary care focused on chronic disease management, dementia care, and preventive health needs
• Develop and manage individualized care plans based on each patient’s needs and goals
• Perform cognitive screenings, Annual Wellness Visits, follow-up visits, and hospital-to-home transition care
• Coordinate care with specialists, home health, hospice, DME providers, and community agencies
• Deliver compassionate, patient-centered care directly in patients’ homes throughout the assigned service area

What You Bring

• Active Virginia Nurse Practitioner (NP) or Physician Assistant (PA) license
• DEA registration or eligibility to obtain
• Medicare PECOS enrollment
• Experience in primary care and geriatrics
• Ability to work independently in a field-based environment
