Case Manager Supervisor

The IDD Targeted Case Management Lead provides support to Oakland County youthwho receive Applied Behavioral Analysis (ABA) andIDD (intellectual and/or developmental disability) services, including waivers(HAB, SED, CWP, etc). This position oversees a team of case mangers that provide youth and their family with assessment, supports coordination as identified within the person-centered planning process. Essential Functions Ensures TCM team(s) maintain active caseloads of individuals- meeting the needs of the community and the sustainability of the team; including holding a consistent caseload of individuals. Is the primary contact for receiving logging and tracking case management referrals. Primary contact for emergencies that occur within the team. Responds immediately and effectively to deescalate situations and report appropriate occurrences to supervisor, law enforcement, and social services. Monitors individual and team performance. Documents and remedies accordingly. Provides and ensures appropriate coverage of intensive supportive interventions. Facilitates development of a Wraparound Plan utilizing Family Center Planning, Family Driven and Youth Guided Care principles with the child/youth, family and family team. Monitors implementation of the Wraparound Plan for each assigned family; this includes revising or developing goals and monitoring progress in objective attainment. Works in cooperation with OCHN Autism diagnosticians for recommendations, referrals, and case consultations. Assists families applying for SED Waiver services and assisting in providing all necessary documentation to the MDHHS and PIHP to access services. Ensuring timely completion of SED Waiver renewal process. Entering and maintaining current (IPOS) information in the WSA system. Collaborates with local DHHS . Assisting with the identification and recruitment of Waiver providers for therapeutic foster care, therapeutic camps, recreational and art therapies. Establishes crisis plans and responds to client requests for mental health coaching or emergency counseling by phone or face to face as determined by Wraparound Plan. Completes accurate documentation with the OCHN Electronic Health Record System within 24-hours of service delivery. Communicate effectively in written and verbal form with Individuals, families, providers, and community partners to ensure coordinated, person-centered care. Leeds weekly in case reviews / supervisions with case managers. Active participant in the Behavior Treatment Plan Review Committee (BTPRC) Other duties as assigned. Education Requirements Degree: A master's degree or higher in social work, psychology, counseling, or a related license granting human services field Training Requirements (licenses, programs, or certificates) Certification(s): QIDP, MichiCANS, CWP, ICCW, All HCBS required trainings for OCHN and or MDHHS Recommended: BTPRC, self-directed services, due process,knowledge and training in behavioral analysis and BCBA certification, Devereaux Early Childhood Assessment (DECA) Experience Requirements Minimum 2 years' experience in the examination, evaluation and treatment of minors and their families. Ideally Minimum of 1 year experience in a supervisory capacity Job Specific Competencies/Skills Familiarity with behavioral health and IDD screening tools and neurodivergent diagnostic processes. Strong verbal and written communication skills Interpersonal skills to work with people of a broad range of socioeconomic and cultural backgrounds. Ability to establish and maintain rapport. Demonstrated ability to work effectively in a team environment, as well as independently. Ability to utilize Motivational Interviewing, as well as trauma-informed practices. Problem-solving skills to identify shortcomings of care plans and revise them accordingly. Familiarity with basic pc software, spreadsheets, electronic record management programs and other software. Organizational skills to plan patient appointments and meetings and to coordinate the activities of medical providers. Work requirements include meeting individuals in the home, community, hospital, school setting, and / or office setting.
