Job Summary The Cardiovascular Health (CVH) Unit is seeking a collaborative and innovative public health professional to help improve cardiovascular health outcomes across Minnesota through clinic and community-based partnerships. This position provides clinical expertise and leadership to support systems change efforts focused on preventing and managing cardiovascular disease, hypertension, high blood cholesterol, and related cardiometabolic conditions. Working within the Health Promotion and Chronic Disease Division, this role partners with clinics, health systems, community organizations, and mental health providers to implement evidence-based strategies that improve patient care, reduce barriers to treatment, and advance health equity for populations disproportionately impacted by cardiovascular disease. This position leads and supports quality improvement initiatives that strengthen care coordination, promote team-based care, improve medication adherence, expand community-clinical linkages, and increase participation in evidence-based interventions. The role also supports statewide cardiovascular health priorities through technical assistance, professional education, data monitoring and reporting, communications, and cross-program collaboration with initiatives such as SHIP, Sage, and other CDC-funded programs. Primary responsibilities include:

- Building and strengthening partnerships with clinics, health systems, community organizations, and behavioral health providers;
- Supporting cardiovascular quality improvement initiatives and implementation of evidence-based practices;
- Analyzing, monitoring, and reporting clinical data to improve patient outcomes;
- Advancing community-clinical referral systems and patient support services;
- Integrating health equity principles into cardiovascular health strategies and partnerships;
- Providing technical assistance, training, and education to healthcare and allied health professionals;
- Assisting with grant management, reporting, evaluation activities, and implementation of Minnesota's Action Plan to Address Cardiovascular Disease, Stroke, and Diabetes 2035.
Qualifications Minimum Qualifications Four (4) years of experience in public health, healthcare, clinical systems, or community health settings working on chronic disease prevention, management, care coordination, or quality improvement initiatives. Experience must include each of the following:
- Experience working with healthcare systems, clinics, hospitals, community health organizations, or other clinical/community-based partners to implement or support health improvement strategies.
- Experience coordinating projects or initiatives involving multiple internal and external stakeholders, including planning, implementation, monitoring, reporting, or evaluation activities.
- Experience using clinical, public health, or population health data to support quality improvement, systems improvement, program implementation, monitoring, or reporting efforts.
- Demonstrated ability to build and maintain collaborative working relationships with diverse partners, including healthcare providers, community organizations, and multidisciplinary teams.
- Ability to communicate complex public health, clinical, or health equity concepts clearly and effectively to a variety of audiences.
- Demonstrated facilitation, consultation, and technical assistance skills, including the ability to support partners through change management or quality improvement efforts.
- Ability to prioritize and manage multiple projects, deadlines, and competing priorities while working independently and collaboratively in a fast-paced environment.
- Advanced degree in Public Health, Nutrition, Epidemiology, Public Health, Health Care Administration, or related field
- Experience supporting or implementing quality improvement (QI) initiatives within healthcare systems, clinics, Federally Qualified Health Centers (FQHCs), or community-based health settings.
- Experience integrating health equity, social determinants of health, or community-clinical linkage strategies into public health or healthcare initiatives.
- Experience providing technical assistance, training, facilitation, or education to healthcare professionals, community partners, or multidisciplinary teams.
- This position requires successful completion of a background check and employment reference check.
