Job Details

We're seeking an experienced Hospice Nurse Team Leader (RN) to manage a dedicated hospice team in Manhattan. This hybrid role allows you to lead from our 55 Water Street office 3 days a week and spend 2 days in the field providing direct care to a limited number of patients.

Key Responsibilities:

Oversee an interdisciplinary team and ensure delivery of high-quality hospice care

Serve as a clinical mentor and administrative leader to nursing staff

Manage care coordination, compliance, and documentation

Conduct patient visits on Thursdays and Fridays, maintaining a small caseload (8-10 patients)

Collaborate with physicians, social workers, chaplains, and support staff
