PACE Home Care Coordinator - Senior Care & Therapy Services

Miami Jewish Health is seeking a Home Care Coordinator to develop and implement homecare services for Florida PACE Center participants, coordinating nursing, therapy, and Durable Medical Equipment to meet client needs. Experience with frail elderly populations, 1–3 years in home care, and a valid Florida driver’s license are preferred.
