About the Role: As a Registered Nurse with Concord Care of Hartford, you'll have the opportunity to work in a dynamic and supportive environment that prioritizes staff appreciation and job satisfaction. You'll work closely with our interdisciplinary team to deliver high-quality patient care, develop individualized care plans, and provide emotional support to our residents and their families. Responsibilities: Provide direct patient care, assessing patients' physical and emotional needs, and developing effective care plans Collaborate with other healthcare professionals to ensure seamless care transitions and coordinate patient care Educate patients, families, and caregivers on disease management, medications, and treatment plans Maintain accurate and detailed records of patient care, including assessments, treatments, and medications Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care Demonstrate compassion, empathy, and respect for patients, families, and colleagues Shift Requirements: We currently have the following shifts available: 1 full-time shift from 6:00a-6:30p 1 part-time shift from 6:00a-6:30p 1 full-time shift from 6:00p-6:30a (night shift) What We Offer: At Concord Care of Hartford, we prioritize our employees' well-being and offer a range of benefits to support your physical, emotional, and financial health.
