This role requires a Registered Nurse (RN) or an individual with an advanced clinical degree. How you will make an impact Business Development & Outreach: Proactively grow the organization by identifying new opportunities, building strong referral relationships, and representing the company in the community. Patient & Family Engagement: Act as a trusted, compassionate resource, educating patients and families about hospice services and guiding them through enrollment. Communication & Coordination: Strengthen partnerships with healthcare providers, ensure smooth communication between referral sources and clinical teams, and manage timely follow-ups and documentation. Market Leadership: Drive awareness and growth by educating stakeholders, analyzing referral trends, staying informed on market dynamics, and positioning the organization as a trusted leader in the community. The Experience and Skills You’ll Bring Strong verbal and written communication skills with ability to engage clinical and non-clinical audiences Effective relationship-building skills with referral sources, patients, and interdisciplinary teams Customer service–oriented with strong active listening and professionalism Ability to educate and influence using a consultative, clinically informed approach Working knowledge of medical terminology, healthcare delivery systems, and referral processes Familiarity with electronic health records (EHR); hospice and palliative care knowledge preferred Strong organizational and time management skills with ability to manage multiple priorities Proficiency in Microsoft Office and ability to learn new systems quickly Demonstrates initiative, adaptability, and ability to work collaboratively in a team environment Associate’s degree in nursing (ADN) or Bachelor of Science in Nursing (BSN) Active, unrestricted Registered Nurse (RN) license in the state of practice. Preferred Knowledge, Skills and Experience Knowledge of healthcare referral processes and case management Experience with hospice care and understanding of prognostic indicators Two (2) years of clinical nursing experience, including case management in a clinical setting #J-18808-Ljbffr Hospice Acquisition Company LLC
