Physical Therapist (Home Health)

Shift: 8:00 AM - 4:30 PM
Job Description
We are seeking a dedicated Physical Therapist with a minimum of 2 years of experience in home health. The ideal candidate will have demonstrated expertise in start of care and revisit assessments. This role involves delivering high-quality physical therapy services to patients in their homes.
Responsibilities
- Conduct comprehensive assessments and evaluations of patients' physical abilities and needs.
- Develop and implement individualized treatment plans to improve patients' mobility and overall health.
- Perform start of care and revisit visits as required.
- Document patient progress and modify treatment plans as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Required Experience / Certifications / Licensure
- Minimum of 2 years of experience in home health.
- Active Physical Therapist license in Pennsylvania.
