Licensed Practical Nurse (LPN) Evenings
Job Summary:
Wilkinson Residential Health Care Facility is a leading healthcare company, seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Amsterdam, NY. As an LPN with our facility, you will play a vital role in providing high-quality patient care to our residents while working evening shifts. Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and vital sign checks.
Educate patients and families about disease treatment plan, including self-care post discharge.
Monitor and report changes in resident condition to the nursing team and attending physicians.
Assist with resident transfers, ambulation, and mobility exercises.
Assures resident safety by identifying safety hazards, emergency situations and initiating corrective action as appropriate.
Maintain accurate and up-to-date records of resident care and treatment.
Participate in quality improvement initiatives to promote excellent patient outcomes.
Collaborate with other departments to ensure seamless delivery of care.
Requirements:
Currently licensed as a Licensed Practical Nurse (LPN) in the State of New York.
Must be able to speak, read and follow instructions in English.
BLS Provider obtained with within 3 Months (90 days) of hire date or Job transfer date.
Ability to work variable shifts, including evenings and weekends.
Strong communication and interpersonal skills.
Basic computer skills and familiarity with electronic medical records.
Compassionate and caring attitude towards patients and their families.
