Full-Time Home Health RN Case Manager (On-Call)

Otterbein SeniorLife is seeking a Registered Nurse (RN) - Case Manager to join our Home Health team in Ohio. You will perform initial assessments, develop individualized care plans, and coordinate with physicians to deliver patient-focused, compassionate care in the home setting.
