Work Environment

This position is entirely onsite, as you will be delivering home care to our patients. Your work environment will include a diverse range of client settings, from private residences to community-based settings within Port Charlotte. This close-knit atmosphere allows you to build strong relationships with your patients while utilizing your skills to provide personalized care. The work schedule is Monday through Friday from 8:00 AM to 5:00 PM, allowing for a work-life balance that is essential in the healthcare profession.

Key Responsibilities

• Conduct comprehensive examinations and evaluations to identify functional limitations and disabilities in patients.
• Design and implement individualized treatment plans that align with the latest best practices in physical therapy.
• Utilize state practice acts effectively to ensure compliance with laws and guidelines that govern physical therapy practice, including supervision of therapy assistants when necessary.
• Perform thorough documentation of patient assessments, treatment protocols, progress notes, and discharge summaries to maintain accurate patient records and ensure continuity of care.
• Consult with a multidisciplinary team of healthcare professionals to enhance patient outcomes and modify care plans based on observed changes in patient conditions.
• Participate in discharge planning and follow-up care, ensuring that patients achieve their therapeutic goals and are equipped to maintain their health independently.
• Engage in ongoing education and training to stay abreast of evolving best practices and compliance requirements in healthcare.
• Actively participate in team meetings, case discussions, and professional development opportunities to elevate service delivery and patient care.
• Contribute to organizational assessments of service accessibility and client satisfaction by analyzing and engaging with key performance indicators.

Patient Population / Client Focus

In this role, you will work predominantly with patients requiring rehabilitation for a variety of conditions, including post-surgical recovery, chronic pain management, neurological disorders, and musculoskeletal injuries. Your expertise will aid diverse age groups, from pediatric to geriatric, many of whom will benefit from personalized, in-home physical therapy interventions that promote their independence and functional abilities.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• A valid state license to practice physical therapy in Florida.
• Current CPR certification to ensure safety and preparedness in emergency situations.
• Excellent interpersonal and communication skills to effectively build rapport with patients and their families.

Preferred Experience

• At least two years of experience in a practice setting relevant to physical therapy, such as home health or outpatient care.
• Experience with digital health applications or solutions that enhance therapy delivery would be advantageous.

Compensation and Benefits

Mobility Works Physical Therapy offers a competitive annual salary ranging from $100,000 to $120,000, commensurate with the candidate's experience and qualifications. In addition to the base salary, our comprehensive benefits package includes:
