RN Territory Clinical Manager, Home Health

divh2Home Health Care Manager/h2pSupervise day-to-day clinical and office operations; remain available to staff and patients during business hours./ppEnsure all services meet professional standards, agency policies/procedures, and state/federal regulatory requirements./ppCoordinate patient referrals and care with the interdisciplinary team./ppOversee the development, implementation, and updating of individualized plans of care./ppEvaluate the quality of clinical care and continually assess patient needs./ppCollaborate with clinicians and physicians on patient transfers, discharges, or changes in condition./ppManage OASIS documentation, edits, discharge processes, and related data./ppMonitor key performance indicators (KPIs), workflows, and escalations./ppGuide clinicians on emergency care protocols./ppProvide training, supervision, coaching, and mentoring to clinicians (including contract staff); assign patients/staff based on competence./ppAct as QAPI (Quality Assessment and Performance Improvement) owner; participate in QA activities, investigate incidents, analyze trends, and implement corrective actions./ppSupport accreditation readiness, QA audits, HHCAHPS surveys, and emergency preparedness planning./ppServe as backup for other roles (e.g., Patient Care Manager) and act as leader in the absence of the Territory Administrator./ppManage operational aspects in collaboration with the team to support exceptional home health care./ph3Qualifications:/h3ulliMust be RN/liliMust have HH Leadership experience/liliMust have OASIS experience with HCHB experience preferable/liliWill be leader in absence of Branch Director/liliSome duties include: CHAP/Survey readiness, QAPI owner, ensure implementation of agency policies and procedures, QA audits, etc./liliDoes not have a team of clinicians reporting directly to them/li/ululliGraduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation./lili3-5 years experience as a RN./liliExperience managing RN services in the home, community, or clinic is preferred/li/ulh3Required Certifications and Licensures:/h3ulliLicensed to practice as a registered nurse in the state of agency operation./liliMust possess and maintain valid CPR certification while employed in a clinical role./liliMust be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
