Job Summary:

Seeking a licensed Physical Therapist for a home health field position in the Central Valley, Stockton, or Modesto area. The role involves providing skilled outpatient therapy services to patients in their homes, with flexible scheduling options and a focus on quality care.

Responsibilities:

• 
  Administer comprehensive physical therapy assessments and develop individualized treatment plans.
  
  
• 
  Deliver therapeutic interventions tailored to patient needs, including exercises and mobility training.
  
  
• 
  Maintain accurate, timely documentation of patient progress and treatment activities.
  
  
• 
  Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
  
  
• 
  Adhere to all safety, compliance, and regulatory standards, including travel requirements.
  
  
• 
  Communicate effectively with patients, families, and healthcare providers to coordinate care.
  
  

Qualifications:

• 
  Hold an active California physical therapy license or be in the process of obtaining it.
  
  
• 
  Possess at least 1 year of experience in home health therapy or relevant clinical practice.
  
  
• 
  Demonstrate proficiency with Oasis documentation and home health electronic systems.
  
  
• 
  Maintain a valid drivers license and personal transportation for patient visits.
  
  
• 
  BLS certification through AHA is required.
  
  
• 
  Exhibit excellent communication skills and timely documentation habits.
