This position offers a base salary of $120,000.00 and a competitive benefits package including medical and dental insurance, 401k Plan, and 35 PTO Days. Our agency has won numerous awards for employee satisfaction and patient care./ppstrongThe Clinical/Nurse Manager oversees, assesses, plans, implements, and evaluates the nursing process and has responsibility for:/strong/pulliConducting on-site supervision and evaluation of staff to ensure the delivery of quality patient care/liliOrienting all nursing personnel to the companys clinical policies and procedures as well as to each home care case/liliSupervising field staff under appropriate contract requirements/liliCoordinating home care services/liliAssessing ongoing educational needs of staff and coordinating necessary training/in-service programs/liliConducting in-service programs/liliEnsuring patient care is coordinated and managed appropriately at all times/liliEnsuring that care and services are delivered appropriately and for the supervision of clinical personnel/liliEnsuring progress reports are being properly documented and in compliance with state regulations/liliParticipating in quarterly supervisory visits and rotating on-call weeks in order to troubleshoot patient issues/liliAssisting in scheduling for clinicians on assigned team/liliEnsuring accurate and timely documentation and transmission of reports from clinicians/li/ulpstrongRequirements:/strong/pulliNYS Registered Nurse/liliKnowledge of Oasis documentation/liliExperience and knowledge of CMS/DOH Home Health Rules and Regulations/liliExperience and knowledge of Home Health Policies and Regulations/liliNurse Manager experience required/liliStrong clinical skills (IM/Sub-Q injections, Foley catheter care, ventilator trache care, infusion nursing, wound care, medication management, PEG tube care)/liliExperience in coordination of care, doctors orders/485s and clinical documentation for CHHA required/liliFamiliar with new COPs and PDGM rules/liliMinimum 2 years clinical experience/liliRN with 5+ years of home care and at least 1 year supervisory/management experience/liliIdeal candidate will be a team player with a positive attitude, good organizational and time management skills, good business sense, and strong problem-solving skills/liliExperience with electronic medical records, experience with Netsmart is a plus/liliMust be able to meet assigned goals, understand the delivery of service within the home care setting/liliExperience in CHHA Conditions of Participation as well as experience in utilization management and quality standards/liliCompany provides excellent benefits, PTO, compensation, health insurance/li/ulpBenefits:/pulliFlexible schedule/li/ulpSchedule:/pulli8 hour shift/liliMonday to Friday/li/ulpWork setting:/pulliIn-person/li/ulpLicense/Certification:/pulliRN License (Required)/li/ulpWork Location: In person/p/div
