What You Will Do:

• Lead comprehensive hospice case management services by assessing patient needs, developing care plans, and coordinating timely clinical interventions.
• Partner closely with physicians, families, caregivers, social workers, and interdisciplinary team members to promote compassionate, patient centered care.
• Support effective care coordination across home health, assisted living, skilled nursing, memory care, and community based settings.
• Monitor clinical status, document care accurately, and help ensure services align with care management, utilization management, and clinical operations standards.
• Educate patients and families about palliative care options, symptom management, community resources, and available support throughout the care journey.
• Contribute to patient satisfaction by communicating with empathy, responding proactively to concerns, and advocating for each patient’s goals and preferences.

What You Will Bring:

• You bring strong registered nursing and hospice case management experience, with the clinical judgment to prioritize evolving patient needs.
• You hold a Bachelor of Science in Nursing or possess comparable nursing education and experience relevant to complex care coordination.
• You communicate clearly and compassionately with patients, families, providers, and interdisciplinary care teams during sensitive situations.
• You have demonstrated knowledge of care management, discharge planning, clinical documentation, and coordinated transitions of care.
• You are organized, accountable, and comfortable managing a caseload while maintaining high standards for quality, service, and patient safety.

Preferred Qualifications:

• Certified Case Manager credentials or experience in utilization review, managed care, utilization management, or InterQual criteria are valued.
• Experience supporting patients in hospice, palliative care, assisted living, skilled nursing, or memory care environments is a plus.
• Previous nursing management, performance evaluation, or clinical operations experience will help you thrive in this patient focused role.

Compensation and Benefits:

• This Hospice RN Case Manager position offers an annual salary of $80,000 to $88,000, based on qualifications and experience.
