Clinical Social Worker (40 Hour) (Office/On-site)

Introduction We are seeking a compassionate and dedicated professional to support individuals on their recovery journey from mental health and substance use challenges. Position Clinical Social Worker (PCN 126186) Location: Connecticut Valley Hospital (CVH) Adult Services Division (ASD) in Middletown. Schedule 1st shift: 8:00 AM – 4:30 PM Full‑time, 40 hours per week Sunday – Thursday Benefits Access to a comprehensive suite of state employee benefits. Key Responsibilities Provide discharge planning and psychosocial support for patients in an acute psychiatric hospital setting. Complete thorough assessments to identify clinical needs and inform individualized treatment planning. Synthesize patient history, clinical observations, and evaluation data to develop professional recommendations. Maintain clear, accurate clinical documentation that meets agency and regulatory standards. Collaborate with interdisciplinary teams and community partners to ensure safe, timely transitions and support recovery. Qualifications & Requirements Licensure as a clinical social worker in the State of Connecticut. Knowledge of social work methodology, casework, group work, community mobilization, and related ethical principles. Strong interpersonal, oral and written communication skills. Ability to devise and implement treatment plans with measurable goals that address client needs. Independently apply current psychiatric treatment modalities. Additional Requirements May be required to have knowledge of sign language and communication ability with the deaf and hearing impaired in certain designated positions. May be required to have ability in Spanish oral and written communication in certain designated positions. May be required to travel.
