Clinical Nurse Coordinator Transitional Care Unit

Methodist Hospital, a leading healthcare provider in San Antonio and part of SA Health, is seeking a Clinical Nurse Coordinator for our Transitional Care Unit. In this key leadership role, you will coordinate and oversee the delivery of high-quality, patient-centered nursing care for adult patients who are transitioning from acute hospital care toward discharge, rehabilitation, or longer-term care settings. You will function as a working RN at the bedside while also serving as a clinical leader, resource, and mentor for staff on your shift. The Clinical Nurse Coordinator is responsible for managing daily patient flow on the Transitional Care Unit, including admissions, transfers, and discharges, and for ensuring that each patient receives safe, evidence-based, and timely care in accordance with hospital policies, regulatory standards, and best practices. You will actively participate in interdisciplinary rounds, collaborate with physicians, case managers, therapists, and social workers, and support the development and implementation of individualized care and discharge plans that promote optimal recovery and reduce readmissions. Key aspects of this role include supervising and supporting nursing staff, providing clinical guidance during complex or rapidly changing patient situations, and acting as a role model for compassionate, culturally sensitive care. You will assist with staff assignments and workload balancing, help monitor staffing levels and acuity, and elevate needs to leadership as appropriate. You will be expected to precept new nurses, support ongoing staff development, and champion quality and safety initiatives on the unit, including infection prevention, falls reduction, medication safety, and documentation accuracy.
