Responsibilities:

• Provide psychosocial assessments and develop individualized care plans for residents

• Identify and address the social and emotional needs of residents, including those related to mental health, trauma, and aging

• Develop and maintain relationships with residents, families, and healthcare teams to ensure comprehensive care

• Coordinate with outside agencies and community resources to access support services for residents

• Monitor and report on resident outcomes and program effectiveness

Requirements:

• Associate's degree in Social Work or related field

• 1+ years of experience in a social work setting
