Shifts: M-F 8am-5pm (No oncall)

Seven Healthcare is seeking an experienced RN Case Manager for a travel assignment in Waterford, WI. This role specializes in hospice care and requires expertise in wound care. The environment involves visits to clients in Kenosha, Racine, and Southeast Milwaukee County. Candidates must have a strong background in home care and electronic medical records systems.

Key Responsibilities of the RN Case Manager

Perform comprehensive assessment of high-risk members
Collaborate with primary care providers to implement care plans
Identify and reassess cost-efficient, appropriate levels of care
Coordinate non-medical support for treatment plan compliance
Coordinate care transitions and collaborate with healthcare professionals
Engage specialty resources as needed for optimal resolution
Maintain detailed records of management process outcomes
Identify health promotion and illness prevention opportunities
Act as a patient advocate by ensuring privacy and confidentiality

Requirements of the RN Case Manager

RN License for WI
Hospice experience, Wound Care Experience
Home Care Home Base (HCHB) experience desired or similar EMRs acceptable
Certified CPR/BLS
Valid Driver's License and Reliable Vehicle
Valid Car Insurance
Completion of skills competency checklist
Elder Abuse, HIPAA, OSHA, Infection control training
Associates or Bachelor's Degree in Nursing or related field
Case Management certification
