SHPCA SCAN Health Plan
SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for SCAN members in their residences, primarily in senior living communities and skilled nursing facilities. You will perform exams, manage chronic diseases, administer vaccines, and collaborate with care teams to prevent hospitalizations.
