Work Environment\n

This position allows for a remote working environment, providing flexibility while engaging with clients and their families. The primary work setting will be within a school environment, facilitating assessments and interventions for seniors who may be involved in educational programs or community outreach initiatives. Work hours are Monday to Friday, from 8:00 AM to 4:30 PM, though flexibility is necessary to accommodate clients’ needs, which may occasionally require evening or weekend hours.

Key Responsibilities\n

• Conduct in-depth assessments of seniors' psychosocial needs, focusing on their emotional, mental, and social health.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring that they align with individual goals and circumstances.

• Provide counseling and emotional support to seniors and their families, assisting them through life transitions, such as relocation, loss, and caregiver challenges.

• Advocate for clients' rights, helping them gain access to essential community resources and support services, thereby enriching their quality of life.

• Work in harmony with an interdisciplinary team, which includes healthcare professionals, caregivers, and educators, to coordinate comprehensive care for our clients.

• Perform regular visits to clients, assessing their well-being, living conditions, and identifying any areas of concern that need addressing.

• Facilitate communication and conflict resolution among family members involved in the care of senior clients to foster supportive environments.

Compensation and Benefits\n
